Internet Physical Therapy – Is It Possible?

Once upon a time, Physical Therapy was a tech position, where doctors would prescribe a set of exercises and have the therapist carry them out. There was no freedom of thought, no ability to modify the plan of care, and no education to support their suggestions.

Today is a very different day. Today, we live in a world where there are Doctors of Physical Therapy – true experts in our field. Autonomy is a privilege and a right for Physical Therapists, and ‘Direct Access’ even exists. This means that you can see a Physical Therapist in the United States without even having to see a physician first.

We are blessed to be in this position, as Physical Therapists are able to provide recommendations and plans of care that far exceed the limited amount of training that medical doctors receive in regards to exercise program design, pain relief through non-pharmaceutical means, and movement abnormality diagnosis. You see, the way you move may be causing you harm or it may be helping you, but first you need to understand what’s right, and how your body, in particular responds to each exercise.

I regularly receive comments and feedback from people who have trouble with the following exercises, above all the rest:

  1. Squat
  2. Lunge
  3. Pull Up
  4. Push Up
  5. Deadlift
  6. Cleans
  7. Presses
  8. Planks

Understanding that these 8 exercises are the eight most fundamental exercises for fat loss, this has left me distraught. In person, we’ve developed a program called ‘Metabolic Training’ (more to come on this soon) that helps overcome these types of issues. However, we’ve never been able to achieve this online.

  • I would be interested in knowing and finding out more information about becoming one of your Master Therapist. I was a professional dancer ofr 15 years and then got certified as a personal trainer from UCLA.  As well as teaching water aerobics. I have been out of the fitness field for many years but would be extremely interested in getting back into the field and to help other achieve their fitness and weight lost goals.

  • I just fired my personal trainer at the gym, as she didn’t get that I can no longer afford to ignore that I get fibromyalgia, neck spasms, back spasms if I work out too hard.  I asked for “an old guy”, or someone who could “get” that I want to work out “smart” not “quick.  
    It appears you may have a solution to combine smart and quick – love to hear more.

    I would be very, very interested in some help.  I have been through 10 therapy sessions  over a 4 week period with a chiropractor and a physiotherapist for an injury with symptoms that mimic an inguinal hernia.  The pain has been slightly subdued but my activity is still limited.  I am unable to run wind sprints, or ice skate at full speed.   These two activities are pivotal in my fitness program and through staying away from them, I already feel the negative effects on my physical fitness level-lower endurance and strength, slower acceleration, almost no explosive power.  Any advice would be great-Jeff (jeffdrew@rocketmail.com)

    I have learned a great deal by reading your blogs and how focus on strengthening the body through ways that are not conducive to further injury. I have been experiencing hip bursitis and pain in my sacroiliac joint. I do not know how to treat these injuries and would love some advice!

  • I think this is a great idea!  Wish I had seen it about 4 years ago when I fell and damaged my knee.  From that point on I walked (when I walked) differently and very carefully.  I was 63 then.  I refused to do any sort of exercise because it was SO painful.  I kept thinking that I would be fine in a month or so.  It took about 4 years and in the process my other leg/knee/hip is now giving me trouble because I did not exercise for so many years.  The pain went away when I was immobile.  But, of course, that was the worst thing for my weight and my whole body. 

    I am not sure if I damaged my legs/knees more by my inactivity and if it can be repaired with your new program but I would take a look. 

    Funny thing is the more I move the better I feel, the less I move the stiffer I get.

    I have been told to have knee-joint replacement to just accept it as old age.  I refuse to do either.

  • I am interested.  I have intermittent knee pain, ankle pain, and constant shoulder pain (rotator cuff).  I have tried a lot of things, and the docs have not diagnosed anything.  The PT for the shoulder pain has not helped – actually I got better relief and strength from doing your DEFL program.  But I would like to “fix” the ankle and knee.  The knees object to doing your lunge exercises, so I do them very gently.

  • Dr. K,  I have always been drawn to your cutting edge approach to obtaining health through diet and exercise that is far beyond conventional.  This is very exciting as I have certain exercises I avoid due to pain in my elbow that puts pressureon the joint and causes pain.  I have referred my son to your sight also, he playshigh school basketball and experiences hip and shin pain fromt  the constant pounding and running.  He is also frustrated with the uneducated weight training required of them during team workouts that are dangerous and damaging.

    So excited as always to read your newsletters and apply.  Currently, our family of five does your mini spiker set and it is challenging.  Can’t wait to hear about this.

  • My doctor did Not recommend a surgery.
    I have acl partial tear which when I stop workin out for a week or a bit more strengthenin my leg muscles, my tibia goes a bit loose. I also used to have a patellar subluxation and no one could heal me from it. Let’s say, the patella is back into its track currently, by 95%. I did it on my ownby strengthening and conditioning my leg muscles, then my aDductor muscles after, then my m vastus medialis, and that’s the latest thing I’ve reached.
    I have grade II degenerative tear in the posterior horn of the lateral meniscus.
    My knee goes hot when I jog, run, or workout and ‘they’ say I’ve to apply icepacks each single time for a lifetime period or knee effusion might happen.
    One of my shoulder I feel it fricts or clicks. Once I workout verywell, it’s gone. I haven’t worked out for 4 days and here it’s back.
    My lower back has a very very slight damage.
    My waist always gets ‘the beginning’ of an inflammation after 3-4 months
    My right trap is usually declined to the bottom. I try to make both arms liftin and downing exactly the same way. This is what makes the declined line back 95% to normal. I discovered that on my own

    Note: I was lost for 2 years jumpin from a physical therapist to another, until my right knee was about to be damaged cuz I use it alone, without my left leg cuzof da major pain.
    My physical therapist  wanted to train my left leg only and only and only…. saying “I want the nutrition to be directed to those leg’s muscle only to heal faster and grow faster.. and you’ll walk again faster!”
    I trained my right leg and threw this stupid advice to the wall.

    I was really lost and was about to suicide when both of my legs stopped and both arms.
    Now I’m 95% okay in all of my body. Only it replies 100% keepin workin out and moving those muscles

    • I also workout only at home now. Dumbbells and bodyweight and resistance band.
      I’m plannin to get a barbell

      • I have one vein showing in my left calf.  It’s painful.
        When I stand up for 3 mins and – rarely – after 10 mins, both of my groins along with my left calf hurt. It has started since I wore shoes with two rising spots on the side. Two only in each shoe.
        The more I go standing up, my testicles hurt.
        It’s all good as long as I avoid standing and workout. No testicular pain, calf is 50-90% good.
        I don’t have a problem with my testicles. I checked a surgeon

  • I am in my late 50’s and both very overweight and sedentary. I just don’t feel good enough to move a lot of the time. Most of my pains are not paralyzing, just annoying, but I just can’t do the main exercises, I lack the strength and balance. I try some and “dumb” them down to what I can do, but I don’t know if that will actually help me, so it’s hard to stay with it. I’d love to know what I CAN do to get fit within my capabilities.

    You really are amazing.  Do you stay up all night thinking these ideas up? Well you should really get some sleep. 
    Anyway, your idea is fantastic.  I’m looking forward to seeing what you have available for those of us who have difficulty moving,  let alone doing all your fabulous workouts.
    I haven’t given up, but almost there.  So I really need to see what you have in store for those of us who aren’t even close to being physically fit.

  • I hae a chronic pain in shoulder area (begai rom a minor whiplash accident 30 years ago. i also have mild osteoartritis of knees.  This prevents me from doing most of the exercises like squats, lounges, planks etc.  So i have kind ofgiven ypon being able to reduce weight by exercise..

    How wil you do internet physical therapy? We email X ray and other reports and possibly avideo of our problem areas and your questionaire and you work with that?

    What will be the cost?   Is this included in your personal guidance kind of program? 

  • Well, at the moment, I have a pretty painful ankle sprain. I won’t be doing anything for a bit. At least anything with legs. As far as I can remember, my left ankle I’ve sprained many times, walking, hiking, running, playing running sports.
    I’m a little leery of something new and not familiar.

  • Is there something here that would help me with “frozen shoulder”? I’ve been in pain for awhile now, and severe pain for ten months. Finally went to the doctor and I’ve had two cortisone shots this last spring, and worked with a PT (Physical Therapist) since the end of May.
    Concurrently, I’ve been walking every day and have lost 15 pounds.

    I’m seriously considering surgery, but I’m wondering if you might have something better.

    Is this something you think you could help?

  • i’ve had  shoulder blade, scapula and neck pain since a car accident over 10 years ago. i have seen NUMEROUS doctors. specialists, physios, chiros, osteos, acupuncturists, etc, etc, etc and no one can even diagnose my problem let alone cure it and that’s in peerson. I’m sorry but i do not see how my problems could be dealt with over a computer screen. nicwe idea that may work for some ‘normal’ problems but not all.

  • Age— 55 Yrs.Degenerative changes in Spine( one), lumbar region, though mild but still pops up and prevents me from doing heavy dead lifts and heavy cleans.( Docs advised me against wts but I have ignored that and am feeling much better this way) Tendonitis— shoulder, and elbow. But still want to work out and remain fit and muscular.Can you help me? I live in India, Delhi.

    i fell on my right knee and have pain doing other movement other than normal daily ones i also have a weak lower back (prob weak core) so its like rehabilation for me i need  help diet alone is not enuf and gp not bothered

  • I’m a 17th year old teenager. I’m having problem of secondary amenorrhea. I love my healthy lifestyle, working out and eating clean, and I always feel great about it. I already on a good fitness level, love seeing my definition, but it seems that I haven’t been succeeded on the ammenorrhea problem. Just wondering, if it’s my workout that’s too hard for my body, to carry on a good menstrual cycle, or why… Just wondering and that’s now is my main concerned.

  • Oh heck yeah I’m interested.  It is hard to be motivated to exercise regularly or become more active when movement = pain.  I have attempted to resolve the many physical issues I have (with some very real success), but just don’t have the expertise to reach a higher level, even though I have a lot of information.  To be able to access quality therapy would aid my progression exponentially, I’m sure.  Oh to be pain free. what a dream! 🙂

  • I can workout well following your exercises.  My concern is my husband who has a bad back as well as a hip spur that severely limits is ROM.  He is in pain most of the time.  He fell off a ladder many years ago.  Has scar tissue in his gluteus max. and has a history of herniated disc.  I would love to find a way for him to be out of pain and able to workout.

  • Wow, this is a great idea and I hope it’s affordable and accessible for those of us in non-US time zones.  I’m limited mainly by the dessicated/compressed disks in my low back.  They limit my ROM for low back extension and distance running (2K or more) causes severe pain from the pounding.  My hips are pretty locked up from protecting my low back for so long.  I also have plantar fasciitis, less-than-optimum thoracic mobility and had my right ACL replaced in 2006.

    In spite of these limitations, I am in excellent shape and am always looking to improve as I age. I’ll hit the big half century mark in a couple of days and feel I’m only just now entering middle age.  I refuse to give in to “age” and intend to live an active life for another half century or more.

    I recognize that additional limitations may present themselves to me and am looking for guidance to prevent those limitations from even arising.  If they do, I’ll need some guidance around or through whatever comes next.

  • I just had my second meniscal tear a couple of months back and had surgery for it a couple of weeks ago. first one was on my right knee (lateral meniscal tear) in 2004, this one was the same but on my left knee. I am 24 years and the Doctor told me that this kind of injury is far too uncommon for someone my age. Ever since I got the first injury in ’04 I never really ran again… went to the gym and got to a decent level of strenght (in my opinion) for having had an injury to the knee… but here we are again 8 years later. I feel helpless and unconfident about doing the things that I love and even about doing everyday stuff…

  • I’m an outlier, so take my comments with that in mind. I am a 53-year old female who has been extremely fit my entire life. I had a total knee replacement 2 years  ago. Additionally because of extensive scarring, my range of motion (ROM) was getting worse in physical therapy. So I had to have a manual manipulation under anesthesia. My ROM after this procedure was better, but I still lack ROM. I’m functional, but unable to do many of the exercises you recommend. For example, jumping jacks, mountain climbers, and anything plyometric. I continue to strength train, but it’s not like it used to be.

    I find all of the on-line training programs I have tried, NEVER consider my particular situation and generally crank out “canned” workouts for the joint-healthy masses. I’m not saying you don’t have a great program, just that it doesn’t take into consideration limitations (other than conditioning).

    That said, I think online physical therapy would not be something I’m interested in. I’ve had “hands on” PT for various ailments. On-line PT seems like another way to make money, but perhaps I’m cynical because on-line training seems to be mass-produced money makers. The email “touches” are always trying to sell something else, new and better. I know you want to need money and more power to you.

    I know about marketing to your “list” and all of the marketing tricks (I have used them myself). But, I think on-line PT  is risky.

  • I think it is a great idea. Myself having just finished going thru physical therapy from really bad bursitis in my shoulder and having surgery on it. Physical therapy is a great theraputic method to helping out ailments! I still do some of it now to keep things in perspective. Having to go to the appointment 3 times out of my week to alot of my time, going, having the appointment, and then back home again. Your idea sounds great for time reasons in that it is a house call; saving driving time. (more time to concentrate on theraphy). I have no ailments at the time to stop me from reaching my fitness goals except for finding the time to do it for I work late, go to bed late, so that leaves I have to get  up early on little sleep to do it. I deal with IBS in the morning time too. But I will do it for I believe in your program and will figure things out. Again though I just wanted to let you know that I thought it is a great idea! Thanks for everything you do and for the concern you have for all of us out here. Is there anything that can improve on IBS would be great…i don’t know? Just wondering? Of course exercise is good for it on keeping things moving. Thanks!

  • Yes, especially with streaming live video, you can see positions much more easily.  

    I told my students in a few years ( 3 years ago) that cell phones would soon have live video streaming.  My next prediction is holographic video conferencing.  So having a PT available to see and then recommend proper technique is so available.  Won’t be long before there is an App for this too.  

  • I’ve come a long way in the past year with a weight loss of 60 pounds. In late June, however, after a bicycle fall, I injured my left shoulder but x-rays, MRI & CAT scans did not reveal any torn ligaments nor fractures. I would have to wait three months for an appointment for physical therapy! So, with the help of my personal trainer, I kept working on my range of motion and the shoulder is 90% improved.
    Overall fitness training remains problematic, however, since I have had a right knee replacement and I have a foot drop in my right leg. Squats and lunges are a real challenge. I keep dreaming that some day stem cell therapy will help with the leg muscles regeneration for the foot drop, but I don’t seem to be able to do anything about it on my own. I’m working around these issues regardless, but life could certainly be easier if I could get all the core exercises involved.

  • This sounds great. But, next how much is it going to cost.  I am a very picky eater.  I would love to loose some weight.  I also was in a fender bender several years ago, and still off and on do have some middle to lower back pain.
      I also have some shoulder pain that I have been to a sport Dr. don’t know the technical name right now. But, he took  xrays and didn’t see anything.  And still hurts off and on.

  • I’ll share my situation as it isn’t exactly the same as any of the comments I’ve read so far. I am a healthy, fit 49yo female who falls into the category of “starting to think about long-term disease prevention” so I can remain healthy and active for several more decades. I also would love not just to maintain but to improve my fitness level (and okay, look fabulous in my 50s!). However, every time I push myself to new fitness achievements, I end up with a minor injury that sets me back. Sometimes it’s a lower back muscle strain (always on the left side), and most recently it’s a left elbow problem (golfer’s elbow, I think, based on your descriptions of elbow issues). These things never seem like enough to seek out time-consuming, costly treatment, but they frustrate me because I have to scale back my exercise for a while. No exercise = cranky = not good for anyone! I have tried to understand and apply muscle balancing techniques but not sure I’m getting directly at my issue(s). I would love a way to get accurate personal treatment at a reasonable cost (and time-saving, so I don’t miss much work). Thanks!

  • On line work outs that work? Individually geared exercises that are person specific? Why wouldn’t it work? During therapy for whip lash a few years ago, I was not responding to normal treatment….. something was ‘locked’ up tight.  Through Dr. K and associates he works with, I discovered [shoulder injury] something from when I was a child – and the shoulder went into ‘protective hybernation’  from which you need therapy to ‘wake up’ the  muscles.  Fast forward to over 30 years:  Neck, shoulder and upper back pain, NO muscle tone OR response from any upper arm muscle (Turkey arm syndrome deluxe!).   Try to exercise ‘dead’ arms engages overused/overstressed shoulder muscles.  And when one tries to improve self and hurts more – frustration sets in and I read that a lot in the posts below!   

  • Very timely and interesting idea.  I personally have a lot of joint pain and it does make it a challenge to continue with the excerises, knowing the pain will not be lessening thru out the day.  So, what motivates me to cause this pain….knowing if I don’t move these joints and muscles will only make them stiffen faster and the pain will become worse….so let’s see what your program can accomplish in combating these difficulties.

  • I am interested in learning more.  In two weeks I will be having double knee replacement surgery and then weeks of physical therapy.  I am in my sixties, 30 lbs  overweight, and trying to follow you exercise program every morning.  I obviously have trouble with the lunges and squats, but I try to struggle through trying to ignore the pain.  After 10 minutes of your exercise, I do feel better.  I would love to find a way to loose weight and be pain free.  A better quality of life is my main goal.

  • Neck and shoulders–major pain!  🙂  I woke up with a knot and I feel like total yuck b/c of it.  I get a “crick” every so often, but this one has almost made me sick to my stomach.  It’s terrible, and I just got it in my sleep.  🙂  Thanks for all you do. 

  • While this sounds like an interesting program, it is not the first instance of internet physical therapy.  Apparently you aren’t familiar with the Lose The Back Pain program from the Healthy Back Institute (Jesse Cannone and Steve Hefferon).  I have followed their approach with some success.

  • Have issue with right shoulder especially when doing push ups. It seems to be slowly getting better but i know there is something that not quite right with it…Your program and the knowledge you have provided me has helped me lose 24 lbs of pure fat in 5 1/2 months with a injuried shoulder…Will continue to keep grinding it out until i reach my goal but just knowing what is wrong and what’s my solution to repairing it  would be a beautiful thing.   • This is a great opportunity for people to take responsibilty for the course of their treatment.
    If we can stretch or do home exercise with success why not use those same thoughts to move from a pain syndrome to healthy  rehabilitation.  I personally siffer from Dupuytren’s (sp) Contractures in both hands and have often wondered if with the right joint and muscle rehab and release, I might find relief.

  • Dr K, you continually amaze me by your level of caring for all of us old and out of shape people.  We try to struggle through our exercises to lose a few more pounds even with bad knees, backs, shoulders etc.  Now you are going to even help us with our aches and pains to make our work outs easier.  You are just wonderful.

    I have e-mailed you about just such a program!  I have fibro, seizures, and a very poor sense of balance.  I need someone to watch me do the exercises and suggest props or adaptations and tell me when I’m doing them correctly.  I have trouble with squats because of miniscus surgery.  I need someone to help me through the anti gravity and mission unbreakble before I can tackle Full Throttle.
    I am going to lose 30 more pounds and am determined to be healthy and fit so I don’t die early like my parents of diabeties and cancer.  I would be very very interested in such a program and would be willing to help in case you need a giauni pig or suggestions during development.  Thank you so much!

    I am Janet.  18 years ago a suffered a major car accident and received a tramtic head injury.  Needless say I was in a coam for 4 days, life support for 2 weeks , inpatientr rehab for 7weks and 1day, and 3 mo. out patient rehab.  I was and still am a miricle.  My problem is that since I began 4 months pregnant with my 11 year old I started having babinski reflex when I walk.  My toes have moments when they do not curl and all is well , but they curl more than not.  I keep looking for a solution to stop the curling but keep running into dead ends.  If you could help me I would really appreciate this.  I am a stay at home mother and do not have alot of money.  I would really love to walk and run pain free!!

  • That sounds interesting.  I am a moderate exerciser, not good at sports (esp hand/eye coordination), but have always enjoyed exercise–just moving my body, working up a sweat, releasing stress, stretching afterward.  I almost always say to myself afterward, man, I feel so good, relaxed, good about myself.  However, I have a somewhat large appetite, have always struggled with an extra 20 or more pounds.  I  hate dieting, have never done it successfully, don’t see the point in  people restricting themselves, either forever to keep slim, or they yo-yo back and forth constantly.  I’m a nurse and I know that’s not good for you.  I am off work right now (had surgery) and want to exercise regularly. I am over 50 and don’t want to get more sedentary although that is easy to do.  My  problems: 1) I have knee issues from 20-plus years of jogging (mostly stopped several years ago, but jogged a little on a treadmill a few months ago and my knee popped and been tender and bit swollen since.) My knees crunch when I climb stairs, squat or lunge, even though I know that those major muscle moves are important.  2) I intermittently have mild low back pain. I know that when I make a point of doing more ab exercises, that improves, even though I dislike ab exercises the most. 3) I had surgery on my hand last week and so it is in a cast and I can’t put any pressure on it to hold a dumbbell, do a girl’s pushup.  I am able to hold a plank for 20-30 seconds resting on my forearms.  I have less severe issues in the other hand (both thumbs, actually) and so I have, in the last several years, had to hold dumbbells with my fingers and thumbs on the same side, not on opposing sides.  So I’m looking for ways to modify exercises, strengthen gradually, become fitter, burn fat.  I’m afraid of the coming menopausal  weight gain/metabolism changes.      Liz

  • Hi, I´m from Lima, Perú( South America for more signs, ha) I´ve been following your posts and videos, and i´ve been training for a year with very good results. Finally I saw my abs and muscles but two weeks ago i had to stop due to a strange disconfort in my knees. It´s not really pain, just some strange sensation of tension in both internal parts of the knees joints. I hate this, two weeks and i´m gaining fat. I used to do a lot of lunges, squats ( with not really heavy weights ) jump rope and treadmill interval sprints. Well i don´t know, maybe it´s because i used to train every day. Now i´m afraid to come back to an intense workout routine as i used to. I´m loosing my confidence in my own strenght and i´m only 29 years old.

  • Hi, my name is Julie. I am a personal trainer and student physical therapist assistant. Not only am I interested in learning more about your program for my clients and future pts, I originally started following you for myself. I have about 10-15 lbs of fat I’d like to lose, no major injuries or pain (except some thoracic spasms and low back pain), but I lack discipline and motivation at times. That’s my big problem and what is holding me back. This morning is the first day I began to implement positive talking an imagery into my daily routine. We’ll see how it goes. I would love to have an exam and eval performed on me to assess for muscle imbalances and postural deviations so I can implement corrective exercises into my workout. I think what you’re doing is awesome and would love the chance to participate.

  • Interesting idea.  I am 52 years old, have lost 30 pounds since April 1, and optimistically have another 40 to go.  I lost the first 30 pounds with making comfortable changes to my lifestyle (both nutrition and exercise), and with some more focused effort I am confident that I can get the rest of the way, having a body that I can be proud of.  I have a knee that prevents me from working out at the highest intensity level I would like.  I do not have chronic pain, but there is weakness and I am concerned of a more serious injury if I push too hard.  Having access to fitness counseling and physical therapy on-line is intriguing to me.  I enjoy your regular articles and recordings.  Thanks, Mary

  • Sounds really interesting. I am am 32 I have 2 small children and really struggling to getrun rid of leftknee over pregnant belly fat. I really enjoy working out and running. Motivation is not usually not a problem for me but for the last few years I’ve had alot of knee pain. I finally saw an orthopedic surgeon and the xrays showed arthritis already at age 32! Its really frustrating because I want to run and exercise but the pain is getting old and im afraid im doing more damage and am really going to regret it when im older. Im thinking this type of program would help!

  • Very interested in what you’re doing and would love to participate.  I have a left knee that prevents me from walking and exercising like I used to – and I’m gaining weight.

  • It sounds wonderful. However, to sound even better, it needs to be covered by insurance, at least partially. It would be lovely to not need a PCP referral. Also, having female PT/tainers would be great. If that all happens, then sign me up!

    Let me tell you first what my problem is. My name is James and I am from North Bay, Ontario.
    I’m 24 years old and in grade 8 I broke my leg in 4 places. I now suffer from bulges in my spine. But I believe this is due to muscle imbalance. My right foot “which is the one I broke” has less of an arch than my left and a bulge of muscle where the foot arches. Theres a muscle near my chin that bulges out “looks like a knot” when i stand or flex, sometimes even when I walk. Also on my right arm below my elbow the muscle feels a little numb and tingly when i touch it. Because of this I suffer from chronic back pain an deven in my ankle when I stand or walk for too long.  I was wondering if this method of yours will help me in my situation?

  • This sounds exciting and wonderful!!  My main problem is:  As usual, there will be charges for this service and I live on a very small amount of money (Social Security) so I doubt that it would be affordable in my circumstances.

  • It is a really great idea and I can see it could work.  We live an hours drive away from a physical therapist.  It would cost at least $50 in fuel to go there and back although we combine lots of other tasks in our trip.  If its not squillions then we would give it a go and save the fuel money to pay you.

I am interesteed.  I believe your approach has merit but since I bought the program I have has to stop and start twice because my body is aching and I cannot get past it.  It just feels like something is wrong.  I have watched every video and do believe I have an imbalance and have tried to incorporate some of the exercises in your Assess vids but it does not seem to be working.  Very frustrating.   So I am happy to hear there may a way out with your online Physical Therapy.  I hope its not too expensive.  Great work Dr K!   Jill

  • This could be very interesting.  Our base – feet & pelvis, definitely have to be balanced to have a good platform to build upon.  Injuries & insults that have changed the way we move, and the compensations that our bodies have done to adjust to these injuries, does leave many of us with much less than optimal range of motion & function.  Joint replacements play a role for many people.  Some of these will not have full functional resolution but we need to learn the correct way to work with or individual abilities so that we do not further injure ourselves and can enjoy the healthful gains that exercise can provide.  I am recovering from an ankle replacement and now seem to have shoulder impingement from aggravation while using the walker to ambulate for 5 weeks while unable to bear weight on the affected leg.  Many others are affected by life’s insults as well.  It may very well be worth a try to give more access to people that might not be able to get help otherwise.

  • I think this would be fantastic.  I have a bad back and knees from a car accident.  The pain is consant.  When the pain is really bad it is hard for me to get down the stairs to leave the house let alone get to a therapist.  I have canes in both vehicles and also in my home to use as needed.  I have been handling the pain with medication but there are times when even this does not work.

  • hi i ve been diagnosed with patellofemoral pain and find it very difficult to do any lower body exercises or running even cycling is uncomfortable  which is holding me back from reaching my goals. I have had the pain for about 9 months now and it is not getting any better the gp said it is caused by long distant driving and being over wieght i do taxi driving for a living and cant stop that and would love to have a exercise program to help me lose the extra pounds i am carrying as this would be helpful to my knees. If you think you could help me in anyway and have a program that may interest me i would be very grateful,i am open minded and willing to learn and will put in the hard work required i look forward to hearing from you in the near furture thank you for your time Andrew

  • Confusion about how to proceed and what program to do are major obstacles: Lift hard play hard Full throttle fat loss, double edge fat loss, mission unbreakable, antigravity solution, double edge fat loss..where to start? What is the end of the process? Some of the maneuvers are too hard, so how to do them an easier way or what to substitute and how to progress from exercise to exercise to be able to do the exercises that are too hard? Is my form OK to do this exercise repeatedly, or will I hurt myself? Lots of questions left unanswered now. I hope your new system can help with that.

  • To know what muscles/ groups of muscles are not as strong or not working well and how to correct this through excercise and movement would be invaluable. I see a physio who helps ease my pain and gives me some guidelines but it is not enough to ‘fix’ my problems.
    Looking fwd to what you have planned.

  • This sounds very exciting but not so much for myself or my own fitness goals but rather for my adult son who has cerebral palsy and I’d really like to know what I can do to help with his condition. As he gets older his condition worsens and the PT he used to get is no longer available to us and really didn’t make much of an impact anyway. It’s becoming increasingly difficult to bathe and dress him because his muscles have gotten so tight. Is this something you could address or should I be looking elsewhere for suggestions?? 

  • Sounds great!   I’m pretty new to you and your programs… will be 66 in October and, frankly HATE going to the gym.  However, I understand that I need physical exercise and going to the gym is my best alternative.

    I have semi-constant lower back pain originated from back surgery in 1998 to repair a herniated (sp?) disc – and, currently have somehow injured my left shoulder – pain when I lift my left arm above parallel to the ground.

    As I mentioned, I hate going to the gym and use any excuse to miss a day… so, when my back hurts more than usual – and my left arm is hurting, I stay home so as to “… NOT make these ailments worse…”

    To my credit, I normally drag myself to the gym (kicking and screaming)  3-4 times per week (except when these pains are intense – like this past week)  – and, feel that on-line therapy could be very useful to resolve these issues.

  • I am 60 years old and I have to admit that life has dealt me several injuries that limit my ability to do a lot of the fat burning exercises.  My main concern that I would love a program that would help me get up stairs, strengthen the knees, keep my hips mobile, strengthen the core muscles and shoulder muscle flexibility.  This is more of a concern to me than the fat burning aspect of exercise.  My fat burning is something that I feel happens in my kitchen and dining experiences.  I would love to be able to stay active!!  I think that staying strong is the key to that.  I try to take minimal meds for arthritis and I am stalling on any replacement surgery as I do not feel that is the only answer for me.  Help!!!

  • I injured my right shoulder and I  suspected that this could be pulled muscle since  I cannot reach my back just as  far as how I can do it with my left hand.

  • That sounds awesome! I have a lot of pain and arthritis and can’t do most of the eight exercises listed without hurting myself. This makes losing weight quite a challenge. I also have scoliosis, hammer toes and a bunion. I would really like to alleviate the bunion and the associated pain by realigning my body.

  • This all sounds great. I am 56 and am fairly active with some upper back pain. I walk most days for 35 minutes in the morning. I can physically do most of the exercises. I’m having trouble motivating myself to do more.

  • Has great potentional, as usual, however getting people for the motivation to get away from their t.v.
    set is always the problem for some.  It took me about 40 years before I was introducted to Dr. K.’s programmes – 20 lbs gone, hopefully never to return.

  • What’s stopping me from reaching my fat loss and fitness goals is that I live with a family that relies heavily on processed foods and gluten. Diet contributes to quite a large portion (about 40%) of a successful fat loss program yet I am not able to eat ‘healthy’ unless I live alone and have all the money in the world to buy all these organic or whole foods (e.g., grass-fed beef, grass-fed butter, raw milk, etc). When you sell these fat loss programs, tell us to make sure we are either living alone or that the people living with us are accustomed to a ‘healthy’ diet in the sense of the Diet Solution Program.

  • I have knee pain (probably osteo-arthritis) but I also get pain going down from knee to ankles. This mean I have some kind of problem with my knee tendenitis and/or  muscle tightness going from knees to ankle and this may even be the cause of my knee artritis.   Can you help cure it ?

    My wife get swelling around the knees easily even if she has to stand for only 30-60 minutes and then she has a lot of pain too. Can you help cure it ?
